วันนี้ไปบริจาคร่างกายกับสภากาชาดไทยมา รู้สึกว่าน่าจะเอาข้อมูลมาเผยแพร่เผื่อมีคนสนใจค่ะ
Today I went to Thai Red Cross to donate my body. So I think I should share some information if some of you are interested. ha...h.a....ha

When? : working day from 8:30 - 16:30

วิธีการ/Methods

ผู้มีความประสงค์อุทิศร่างกายสามารถยื่นความจำนงได้ 2 แบบ คือ
Those who wish to make body donations can either:              

๑. ยื่นความจำนงโดยตรงที่ ศาลาทินฑัต โรงพยาบาลจุฬาลงกรณ์ โดยกรอกข้อความ
ลงใน แบบฟอร์ม ของโรงพยาลบาล 1 ฉบับ เจ้าหน้าที่จะออกบัตรประจำตัวผู้อุทิศร่างกายให้ไว้เป็นหลักฐาน
1. directly submit their intention form at the Division of Body Donation, Secretary Section, King Chulalongkorn Memorial Hospital. The donor is requested to fill out 3 forms-two of which will be given to the donor while the other will be kept by the official. The official will also issue a donor ID;  or 

๒. ยื่นความจำนงทางไปรษณีย์ โดยกรอกข้อความลงใน แบบฟอร์ม และส่งทางไปรษณีย์โดยจ่าหน้าซองดังนี้
2. submit their intention form via mail. The donor will fill out 3 donation forms, two of which will be mailed. The official will then mail you your donor ID card.

เจ้าหน้าที่จะส่งบัตรประจำตัวผู้อุทิศร่างกายให้ภายหลัง

หลักฐานที่ต้องใช้มีดังนี้

๑. รูปถ่ายหน้าตรง ขนาด 1 นิ้ว จำนวน 2 รูป
๒. สำเนาบัตรประชาชน หรือ สำเนาบัตรข้าราชการ 1 ฉบับ

Required Documents

1. Three 1-inch or 2-inch photos
2. a copy of ID card or civil servant ID card
3. a copy of residence registration

คุณสมบัติของผู้บริจาค/Qualifications of donors

ผู้มีความประสงค์อุทิศร่างกายต้องมีอายุตั้งแต่ 17 ปีขึ้นไป
กรณีที่อายุต่ำกว่า 20 ปี ต้องได้รับความยินยอมจากผู้ปกครองเป็นลายลักษณ์อักษร

Donors must be at least 20 years old. Those who are under 20 must have a written consent from their parents/guardians.

คือโดยส่วนตัวดิชั้นก็ไม่ทราบว่าที่อื่นรับบริจาคหรือไม่ เลยให้ข้อมูลเท่าที่ทราบค่ะ
